We proudly celebrate the dedication of our team members who have reached 10 and 20 years of service at Crossroads Care Kent. Their commitment is invaluable to our mission.
Our Urgent Care Team provides a rapid response service to support carers and prevent the breakdown of a caring situation, which could otherwise lead to a hospital, hospice, or care home admission.
This time-limited service can currently only be accessed by health or social care professionals.
The service is designed to reduce the level of disruption and distress by providing high-quality care for people with care needs and their families in their own homes during times of crisis, or emergency.
Our team of highly trained staff can assist with both practical and personal care tasks, including medication management, nutrition, hydration, and support with daily living and safety.
Providing families with practical and emotional support when caring for a loved one at the end stages of life.
Our Team of experienced care managers will work with families and health professionals involved in their loved one’s care, to determine a safe outcome for them and their family.
Our service involves regular monitoring of the situation, consideration of other services involved, and conducting regular reviews.
Available to carers and their dependents aged 18 and above, Our Urgent Care Team will aim to provide day or night support to meet the need of the family.

Planned appointments (like Hospital, GP, hairdresser), planned surgery for the carer, planned hospital discharge, planned overnight support are not considered an emergency.
Ensure a comfortable chair (ideally not a dining room chair) is available within easy reach of the person with care needs.
No, it is our staff’s responsibility to check on the person with care needs every half-hour so close access is vital.
Our staff require access to toilet, handwashing facilities, and a kitchen to prepare refreshments.
They bring their own food, but if you are able to provide tea/coffee/milk and the use of cups/plates/crockery, it would be appreciated. Any utilised crockery will be washed after use.
Staff will be positioned within close reach of the person with care needs and must not be positioned on a different floor from the person with care need.
Our aim is to provide you with a peaceful night; however, if there are specific circumstances where you would like to be notified, please inform our staff at the start of the visit.
Staff engage in quiet activities to maintain alertness during quieter hours. They'll minimize light usage but may need it for certain tasks to prevent eye strain.
Staff will inquire upon arrival if you wish to be notified in such cases.
Our staff can support a person's care needs with practical and personal care tasks, including but not limited to personal care, medication management, nutrition, hydration, support with daily living, and ensuring safety.
Our staff do not wear uniforms, dress appropriately and carry ID badges for identification.
No. Our Urgent Care service is not pre-bookable. Night cover is only booked on the day.
